LDL cholesterol goal: how low to go in very high-risk patients?

A new meta-analysis shows that very high-risk patients at or below goal gain additional benefit from further LDL cholesterol lowering. This poses key questions for guideline groups: How low should LDL cholesterol goal be set in very high-risk groups? And, how do clinicians identify patients who would benefit most?
